For your initial visit, please bring the following. * Current insurance cards * Form of Identification; driver license or passport for parent/guardian * Prior Medical Records * MRI or CT films/results,EEG tracing/report, labwork evaluations * Medical records from other specialists or primary care physicians * Relevant school and developmental testing and/or neuropsychological assessments If your insurance is a managed care plan that requires a referral authorization to see a specialist, you are responsible for obtaining that referral from your primary care physician. Without a valid referral, the visit will be rescheduled. Kindly, call our office at least 48 hours in advance if you need to change or reschedule this appointment.
